The short answer
Healthcare is one of the better-performing verticals in paid search. Physicians and surgeons average a 12.43% conversion rate and $40.04 per lead, dentists 10.67% and $72.97. The money gets wasted on broad match symptom terms, audience targeting that policy forbids anyway, and tracking that creates HIPAA exposure.
The benchmarks to plan against
The LocaliQ search advertising benchmarks for the period ending 1 June 2026 gives three distinct pictures inside what people loosely call healthcare.
| Category | Avg CPC | Avg CTR | Avg CVR | Avg CPL |
|---|---|---|---|---|
| Physicians and surgeons | $4.76 | 6.61% | 12.43% | $40.04 |
| Dentists and dental services | $8.00 | 5.66% | 10.67% | $72.97 |
| Health and fitness | $6.17 | 5.81% | 6.94% | $67.36 |
| All industries | $5.42 | 6.64% | 8.18% | $66.69 |
WordStream 2026 Google Ads benchmarks data covering April 2025 to March 2026 across 13,474 US campaigns notes that overall cost per lead fell for the first time in five years. Healthcare conversion rates sit well above the all-industry average of 8.18%, which is why a competent account here is usually profitable quickly. The variance is in what you count as a lead: a form fill is not a booked appointment, and a booked appointment is not a kept one.
Policy constraints that shape the build
Two Google policies decide what you can do before any strategy discussion. The Google Ads healthcare and medicines policy requires certification for online pharmacies and telemedicine providers, for addiction services through LegitScript in the US, Canada and selected countries, and for health insurance advertisers in the US. It prohibits promoting prescription drugs without a prescription and speculative or experimental treatments.
The Google Ads personalized advertising policy then restricts audience targeting. Health is a sensitive interest category, which means advertisers cannot use advertiser-curated audiences such as customer match or similar segments for those ads, although predefined Google audiences remain available because sensitive signals are excluded from them.
Read that twice if you have ever been sold a remarketing programme built from condition-page visitors. It is not a best practice question. It is a policy question, and separately a compliance question.
The compliance layer
HHS guidance on online tracking says regulated entities must configure user-authenticated pages so tracking technologies use and disclose protected health information only in line with the Privacy Rule, must have business associate agreements with vendors handling it, and cannot substitute a privacy notice for authorisation. A Texas court vacated the portion treating an IP address plus a visit to an unauthenticated condition page as automatically triggering obligations, and OCR has said it is evaluating next steps (HHS Office for Civil Rights guidance).
Practical build: keep Google tags off patient portals and scheduling flows, use call tracking rather than individual-level web journeys as the primary conversion signal, and import offline conversions from consented, hashed data only after your compliance lead signs off. Coarser attribution is the price of operating legally in this category.
What works
Procedure and insurance queries on tight match types. The buyer searching “dental implants cost near me” or “does [practice] accept [insurer]” converts. The buyer searching “toothache” is reading. Use phrase and exact for the money terms and keep broad match confined to a small, heavily negated experiment budget.
Call-only and call extensions during staffed hours. Healthcare demand converts on the phone. Schedule bids to hours you actually answer, and treat calls over 60 seconds as the conversion rather than clicks.
Location-level campaigns. Separate campaigns by clinic with radius targeting matched to real catchment. A single national campaign for a five-location group wastes budget on the four locations a searcher will not drive to.
Landing pages that answer money questions. Price posture, insurers accepted, next available appointment, parking. Those four facts move conversion rate more than any bidding change, which is why conversion work belongs in the same budget line as media.
Local Services Ads where eligible. Google documents them as pay-per-lead with verification requirements and notes that failing to answer calls or respond to messages can affect ad ranking (Google Local Services Ads documentation). Note that pre-badge ads are not available for healthcare, so plan for the full verification timeline.
What to stop paying for
Stop funding broad match on symptom and condition terms, which delivers researchers and students at clinical-grade prices. Stop building remarketing lists from condition pages. Stop running display and video prospecting for a local practice before search is saturated. Stop counting form fills as leads without reconciling to booked appointments. Stop competitor conquesting on branded drug names, which policy restricts and which converts poorly for a provider.
And stop paying for accounts nobody negates. In healthcare the negative list is the account. Job seekers, students, “how to become”, “salary”, “free clinic”, “near me” variants outside your radius, and every insurer you do not accept.
The report that matters
One row per campaign: spend, calls over 60 seconds, booked appointments, kept appointments, and cost per kept appointment. If your agency cannot produce the last column, the account is being managed on proxies. Reconcile monthly against the practice management system rather than the ad platform, and expect the two to disagree. That reconciliation is the difference between a $40 lead and a $400 patient. It is also the first thing we build in a paid media engagement.
The bottom line
Healthcare paid search converts well above the cross-industry average, so the wins come from discipline rather than cleverness: tight match types, staffed-hours scheduling, location-level structure, compliant tracking, and a report that ends in kept appointments.
